Do you accept low income or sliding scale payments?

Yes.  If your family income is under $60,000 (family of two or more), or less than $40,000 if you are single, you are eligible for sliding scale to see an MFTI.  Feel free to consult with our clinical director to determine your payment amount.

Do you see children and adolescents?

Yes.  We have therapists who specialize with children younger than 13 and we also have a therapist who specializes in adolescents.

Do you do custody mediation work?

Yes.  We contract for child custody mediation independent from the court system.

Are you certified for Alameda County Anger Management classes?

Not at this time.  However, we are currently pursuing this certification so check back in with us.

What type of insurance do you accept?

Aetna – If you have the EAP, HMO, or PPO plans with Aetna, your mental health costs may be covered.  Contact Aetna at (800) 872-3862 or dial the number on the back of your card to determine eligibility.  You will be asked to provide your member ID, group #, and other protected health information to be authorized for benefits.

Anthem Blue Cross– If you have the EAP, HMO, or PPO plans with Anthem Blue Cross, your mental health costs may be covered.  Contact Anthem Blue Cross at (866) 820-0765 or dial the number on the back of your card to determine eligibility.  You will be asked to provide your member ID, group #, and other protected health information to be authorized for benefits.

Blue Cross Blue Shield of California – If you have the PPO/EPO plan with BCBS, your mental health costs may be covered.  Contact BCBS at (800) 393-6130 or dial the number on the back of your card to determine eligibility.  You will be asked to provide your member ID, group #, and other protected health information to be authorized for benefits.

Cigna – If you have the EAP, HMO, or PPO plans with Cigna, your mental health costs may be covered.  Contact Cigna at (800) 433-5768 or dial the number on the back of your card to determine eligibility.  You will be asked to provide your member ID, group #, and other protected health information to be authorized for benefits.

Health Net/MHN – Managed Health Network (Employee Assistance Program- EAP). If you have MHN, please contact member services on the back of your card at (800) 646-9923 to determine eligibility. You will be asked to provide your member ID, group #, and other protected health information to be authorized for EAP benefits.

Kaiser Permanente – If you have Kaiser, please contact the behavioral health screening line at 510-752-1075 for a pre-authorization and let them know you would like to be seen by the office of Dr. Jaseon Outlaw. Note: Couple’s Counseling is rarely covered through the Kaiser Contract. You will be asked to provide your member ID, group #, and other protected health information to be authorized for EAP benefits.

UC Berkeley Student Health Insurance Plan (SHIP) – If you are a UC Berkeley Student, SHIP covers outpatient mental health services such as counseling and assessment, but students will first need to request a referral/authorization from the SHIP office in order to be covered.  Students obtain the referrals initially by contacting Counseling and Psychological Services at 510-642-9494.

United Behavioral Health (Optum) – If you have the EAP, HMO, or PPO plans with United Health, your mental health costs may be covered.  Contact United at (800) 888-2998 or dial the number on the back of your card to determine eligibility.  You will be asked to provide your member ID, group #, and other protected health information to be authorized for benefits.

VOC – (California Victim of Crime Compensation Program).  If you have been involved in a violent crime and that is the reason you are seeking treatment, you may be eligible to receive benefits and get your sessions covered.  Contact VOC at (800) 777-9229 to determine eligibility.

Worker’s Compensation – Contact your adjuster at your worker’s compensation company to determine eligibility and benefits.  We currently accept the following worker’s compensation programs:  York Risk Management Services, Keenan & Associates, The Hartford, WellComp, Hazelrigg, and Federal Worker’s Compensation (Office of Worker’s Compensation Program- OWCP).

If you do not have any of the aforementioned insurance plans, you may contact your insurance company and ask how many “out-of-network” counseling sessions they cover.  Many insurance companies provide out-of-network benefits.  We will also be happy to provide an end of month statement for you as many health insurance companies reimburse their claimants who pay out of pocket.
